I've been in the Frozen Hall's 5mans ~40 times on heroic and have never seen the drop. The limit is per account, and cannot be avoided by changing characters. I squealed a little bit with excitement. About RDF: It is bugged. If not, go out, reset, and try again until you get one of the rares to spawn (Ironspine not so important though, for locks). No essences dropped.
